This form is for students who had attended before fall 1984.
If you attended fall 1984 and after please go to the following web site https://www.accd.edu/Ahomepg.htm San Antonio College.
Student ID is your ssn and PIN is month and year of birth (MMYY). Go to "Login to Student Services" request are submitted to "Official Transcript Request".
Due to a paper shortage only 1 transcripts per request. Please complete then print out, sign and mail or fax this form with a copy of your drivers license (your request will not be processed without proper ID) to:

Requests are responded to within 48 working hours excluding weekends and holidays.
Transcripts are not issued until all accounts with the college are paid, all Admissions requirements are met and copy of picture ID submitted.
